D-SNP Enrollment Guide
Enrolling in a D-SNP follows Medicare Advantage enrollment rules. Timing depends on whether you are new to Medicare, already enrolled, or experiencing a qualifying life event.

Enrollment Periods Overview
You can only join, switch, or leave a D-SNP during specific enrollment periods. The three main types are the Initial Enrollment Period (IEP), the Annual Enrollment Period (AEP), and Special Enrollment Periods (SEPs).
Attempting to enroll outside these windows is generally not allowed unless you qualify for an SEP. Missing deadlines can leave you without desired coverage until the next open period.
Initial Enrollment Period (IEP)
Your IEP is a seven-month window around your Medicare eligibility date—typically the month you turn 65 plus three months before and three months after. If you qualify through disability, your IEP begins the 25th month of SSDI benefits.
During your IEP, you can enroll in a D-SNP if you already have qualifying Medicaid. If you gain Medicaid later, a different SEP may apply. Confirm your Medicaid status before enrolling.
Annual Enrollment Period (AEP)
AEP runs from October 15 to December 7 each year. During AEP, anyone with Medicare can join, switch, or drop a Medicare Advantage plan, including D-SNPs. Changes take effect January 1 of the following year.
Review your plan's Annual Notice of Change (ANOC) before AEP. If your current plan's benefits, network, or formulary are changing unfavorably, AEP is your chance to switch to a different D-SNP.
Special Enrollment Periods (SEPs)
SEPs allow enrollment outside standard windows when certain events occur. Common SEPs for dual-eligible individuals include gaining or losing Medicaid, moving to a new service area, and being released from incarceration.
Dual-eligible individuals also have a quarterly SEP (January–March, April–June, July–September) allowing one plan change per quarter. This provides flexibility not available to most Medicare beneficiaries.
How to Enroll
Enroll through Medicare.gov, by calling 1-800-MEDICARE, through a licensed insurance agent, or directly with the plan. You will need your Medicare number, Medicaid information, and preferred plan selection.
The plan verifies your dual-eligible status before enrollment is finalized. If you do not meet eligibility requirements, enrollment may be denied. SubsidyCheck does not enroll you in plans—we provide education to help you prepare.
After You Enroll
You will receive a member ID card and plan materials. Contact member services to set up care coordination, confirm your providers are in-network, and transfer prescription records.
If you were in a different Medicare plan, coverage typically ends when your D-SNP begins. Avoid gaps by enrolling before your current coverage ends.
Frequently asked questions
Can I switch D-SNPs anytime?
Dual-eligible individuals have a quarterly SEP allowing one change per quarter. Others must wait for AEP or qualify for a different SEP.
What happens if I enroll during AEP but lose Medicaid before January 1?
If you lose Medicaid before your new plan starts, you may not be eligible for the D-SNP. Contact Medicare and the plan immediately to discuss your options.
Do I need to disenroll from Medicaid to join a D-SNP?
No. You must maintain Medicaid eligibility to stay in a D-SNP. The plan coordinates with Medicaid—it does not replace it.
Is there an Open Enrollment Period for Medicare Advantage?
Medicare Advantage Open Enrollment Period (January 1–March 31) allows one switch to another MA plan or back to Original Medicare, but only if you are already in a Medicare Advantage plan.
