Insurance companies decide which counties to serve each plan year. A state with many dual-eligible residents may have dozens of D-SNP options, while rural counties may have few or none. Medicaid integration models also differ—some states encourage fully integrated plans while others have limited D-SNP participation.
Plan availability can change annually. A plan offered this year may not return next year, and new plans may enter your market. Always check current options rather than relying on past year data.
States With Strong D-SNP Markets
States with large dual-eligible populations—such as California, Florida, New York, and Texas—typically have multiple D-SNP carriers competing. This can mean more benefit options and network choices, but also more complexity when comparing plans.
Even in these states, rural counties may have fewer options than urban areas. Your zip code—not just your state—determines which plans are available to you.
Integrated vs. Non-Integrated Models
Some states use Medicare-Medicaid Plans (MMPs) or fully integrated D-SNPs where one organization manages both Medicare and Medicaid benefits. Other states have D-SNPs that coordinate with but do not fully integrate Medicaid services.

Use Medicare.gov's plan finder tool, entering your zip code and confirming your dual-eligible status. Your State Health Insurance Assistance Program (SHIP) can also provide localized plan comparisons at no cost.
Licensed agents can show plans in your area but represent specific carriers. For unbiased comparisons, start with Medicare.gov or SHIP before speaking with agents.
What to Do If No D-SNP Is Available
If no D-SNP serves your county, you can still use Original Medicare with Medicaid or a standard Medicare Advantage plan (if you meet its eligibility rules). Medicaid will continue to help with cost-sharing based on your category.
